It could be gastric problem as well.

Explanation:

The pain is radiating only on breathing reflects it makes space and hence the gas radiates.Had the pain been constant and radiating it could be related to heart and complications, but here the pain is felt mostly while breathing so must be checked for both acidity as well as gastric problems.simultaneously the person must be given an antacid and asked to walk briskly, intake of water should also relieve symptoms. There should be an instant relief within 2-3 minutes and if there is the gas stuck in abdomen must be released by brisk walking.so all this, when the nurse is around, however if the things dont improve,anti-gastric drugs and light easily digesting meals be prescribed.
